MESMAC – Job Description

The Community Mental Health Transformation programme (CMHTP) is a collaboration between NHS organisations, the voluntary and community sector, Adult Social Care, and individuals with lived experience. It aims to transform the organisation and delivery of primary and community mental health services. Leeds Mind, MESMAC and Leeds Survivor Led Crisis Service partner to deliver the Peer Support Service for CMHTP each employing a number of PSWs to cover the city.  

Overall Aims:
To deliver Peer Support interventions to people living with Serious Mental Illness (SMI), complex emotional, social & relational needs in community settings. This includes face to face in their homes, in community venues, and in clinical spaces where available.

Primary Objectives: 
PSWs deliver support through a lived experience model, offering 1:1 and group-based interventions. The delivery model is co-produced throughout the project. PSWs support individuals in accessing community services and link with statutory services, acting as a bridge between primary care and community mental health.  

Main Working Contacts:
Mental Health and Wellbeing Coordinator, Senior Peer Support Worker(s) at LSLCS and Leeds Mind, Team Leaders Leeds Mind, Operations Managers, Finance and Administration staff, MESMAC Board of Trustees, Volunteers/ Interns/ Apprentices. 

Main Tasks and Responsibilities:
The following list of duties provides a general outline of the work of the post holder and is not definitive.  

1. Deliver peer support-based interventions to people with Serious Mental Illness (SMI) / complex emotional, social & relational needs in community settings

2. Work with Senior PSWs to manage their caseloads and time, ensuring they are able to schedule and travel to their appointments.

3. Signpost service users to other relevant services where necessary.

4. Ensure peer support work carried out is recorded on the monitoring system for effective reporting.  

5. Attend and engage in relevant project and clinical meetings 

6. To undertake any appropriate administrative duties.

7. To undertake appropriate training of self as required.

8. To take an active role in supervision and staff meetings.

9. To undertake any other duties which the Yorkshire MESMAC Board/Chief Executive deem to be appropriate to the Project's aims and commensurate with the post.

10. To be aware of Yorkshire MESMAC’s policies and to ensure these are reflected in all aspects of the work of the post holder.

11. To be aware of relevant local, national and regional policies and procedures.

12. The ability to work flexible hours.  This post requires some evening and weekend working.
